Limits:  

1. 1 complete examination every 24 months
2. 1 full mouth x-ray every 24 months
3. 1 recall or specific examination, including polishing and cleaning, every 6 months
4. Oral Hygiene Instruction once every 6 months
5. 8 units scaling or root planning per calendar year (2 units for dependent children under age 13)
6. 8 units occlusal equilibration per calendar year
7. Denture Relines/Rebases/Repairs once every 24 months
8. Bridges and Crowns must be 5 years old to be replaced

Orthodontic Treatment:  
50% Payable

Orthodontic Maximum:  
$1,000 Lifetime

Must be under age 19 to claim Orthodontic benefit.
